Infant /Child Dedication Registration

It is our desire that every parent/guardian dedicating their children have a biblical understanding of the significance of dedicating children to the Lord. Prior to the Infant/Child Dedication Service, parents/guardians are required to attend a Parenting Workshop in preparation or continuance of “training up the child in the way he should go.” In presenting this form to the Administration Department you have agreed to attend the PW. One of our Administrators will be contacting you to confirm the details of your PW session and Dedication Service. Special Notes for Parents and Guardians • Full names of both mother and father of the infant/child being dedicated are required on this form • Parenting Workshops (PW) occurs once a month • Reserved seating is available for immediate family members • An administration fee will be applied for non-members of theREACHcentre

A member is considered: A person(s) who is an active participator of theREACHcentre Ottawa both in Church attendance, financial contributions and Spiritual accountability.
